Change in Coggins (EIA) Test Forms Perforated form replaces standard "yellow" colored form From Ed McKinley DVM, PhD, Director Kord Animal Diagnostic Lab, Nashville, TN The Kord Diagnostic Lab is changing the way we report negative EIA results on the Federal form used by equine owners when they go to horse shows, the Celebration, trail rides, etc. We would like to send this information to horse owners, horse shows, fair operators, trail ride groups, and anyone else who is associated with horses. This particular test and the paper work that results are required to participate in these activities and have been for over 25 years. New Form. The Kord Animal Disease Diagnostic Laboratory (Kord ADDL), with the authorization of the Tennessee State Veterinarian, will soon begin using a new method of indicating a negative EIA or Coggins test result on the standard Equine Infectious Anemia Laboratory Test form, VS Form 10-11. This is the standard "yellow" colored form used by equine owners to verify that testing for Equine Infectious Anemia has been conducted and the test results are negative. This form is needed for interstate shipment of equine species and is often required for trail rides, fairs and horse shows. The new method involves use of a perforator that punches holes in all copies of the form and will indicate: Kord ADDL Date (3-22-10) Neg Using this perforation method will eliminate the need for filling in the bottom of the form labeled "FOR LABORATORY USE ONLY" which required entering Laboratory name, dates, test results, methods, and signing thousands of forms generated each year from laboratory testing. Standard laboratory records still allow for traceability of this information. This method is authorized in Tennessee only for testing conducted by the Kord Animal Disease Diagnostic Laboratory.
